I/we realize that participation in dance classes and activities involves risk of personal injury or death. Despite precautions, accidents and injuries may occur. By signing this release form, I/we (the dancer and parent/guardian) assume all risks related to participating in Backhausdance prerecorded or live virtual classes. Prior to participation, I/we agree to make sure the area is clear of hazards (tables, chairs, etc).  

I/we agree to release and hold harmless Backhausdance including its staff, directors, board, teachers, dancers, guest faculty, and facilities from any cause of action, claims, or demands now and in the future. I/we will not hold Backhausdance liable for any personal injury or any personal property damage, which may occur on the premises, before, during or after classes and performances.

Backhausdance may use my email address to send me information about the company and future classes etc.

This Photo Release Agreement is entered into by and between Backhausdance (“BD”) and the initialing event participant. This Agreement will serve as Participant’s consent to BD photographing and videotaping of Participant’s during the live classes and use of the photography and videotape for internal, archival purposes pertaining to BD’s needs. Participant hereby irrevocably assigns to BD the right to record his/her voice and likeness for the aforementioned needs. Participant acknowledges that any picture or recording taken of Participant under the terms of this Agreement will become the sole and exclusive property of BD in perpetuity. Participant and Participant’s heirs and assigns shall have no right to bring legal action against BD for use of the pictures, recordings or videotape.
